Original patchset from David to allow creation of freetype fonts without using
fontconfig.

This is the original patchset obtained from http://david.freetype.org/cairo/

Since cairo had changed since the patchset was made, some of the patches
required hand-adjustments to cleanly compile in Cairo 1.8.6 -- I'll attach the
resulting patchset which incorporates those edits next.
