That makes little sense to me. Cairo incurs overhead to disobey the
application's intent (why?), and the X server is supposed to incur even more
overhead to undo that again?

Changing RepeatNone to RepeatPad when possible might be a nice optimization in
EXA for apps which don't know to use the latter, but there seems little doubt
that this is a bug in Cairo. If you're refusing to fix it, the appropriate
resolution would be WONTFIX.
