[cairo] 8 bit pseudo color missing.

The lack of 8 bit pseudo-color support in cairo is apparently a real
issue for some people. It makes gtk > 2.6 simply unusable. Owen Taylor
suggested that this could be discussed on this list [1].  So, for the
record, could somebody with intimate knowledge of cairo internals
explain what code needs to be written, and where to hook it up in the
current code?  It would help somebody really interrested in fixing
that to know, at least, where to begin.
