[cairo] New Cairo Qpainter surface

Yep, I can push it out -- cairo-qt.h sounds fine to me, I called it  
cairo-qpainter originally since it was directly using qpainter (e.g.  
like cairo-quartz which is using quartz as opposed to OSX), but either  
one works.  I kept forgetting to send this out, so I'm glad Oleg sent  
it :)
