I've been busy working on other things, so I haven't had a chance to
push this very hard. I'm also still not sure what a good way to
integrate it into cairo is yet. (How much caching, when are the cached
images discarded etc.)

The code is not tied to pixman very much, so it is possible that you
could just include it in poppler for now. If you do go this route I'd be
happy to help with any integration questions.
