[cairo] PDF backend generates images that gs can't display etc

Cairo creates PDF files that only acrobat reader 9 can read. With
acroread 7 I can reproduce the problem.

As I understand it, it is actually a Acrobat bug and a Ghostscript bug
and not a Cairo bug.
